Please find below list of claim documents to be submitted:
1. Insurance policy copy
2. Print out of email intimation (if any or claim ID)
3. Claim Form duly signed by the claimant or family proposer in case of minor
4. Xerox of Health ID Card of claimant.
5. First Prescription / Consultation Letter (Photocopy) from your Doctor.
6. Original Hospital Discharge Card
7. Original pre numbered hospital bill with hospital seal and signature indicating complete details of break up and hospitalization period.
8. Original pre numbered paid receipt with revenue stamp, hospital seal and signature towards the Final hospital bill with complete details of break up indicating hospitalization period.
9. Original or Photocopy of all Laboratory & Diagnostic Test Reports. E.g. X‐Ray, E.C.G, Ultrasound, MRI Scan, Haemogram etc.
10. If you have purchased medicines in cash and if this has not been reflected in the hospital bill please enclose a prescription from the doctor and the supporting medicine bill from the Chemist.
11. If you have paid cash for Diagnostic or Radiology tests and it has not been reflected in the Hospital bill, please enclose a prescription from the doctor advising the tests, the actual test reports and the bill from the diagnostic centre for the tests.
12. Copy of cancelled cheque of the main insured
13. Copy of PAN card –if claim amount is more than Rs.1 lac
